button when located in the tracheostoma. The special design of the
valve base plate and the flexible silicone seal on the button guaran-
tees a reliable and airtight fit between these parts. This also ensures
that the valve is not detached from the button in the event of violent
coughing.
REF 650-S and REF 650-SO may be inserted into and removed
from the TRACOE adhesive carrier (REF 660) in a similar way.
REF 650-T, REF 650-TO and REF 650-TO-C are pushed onto the
15 mm connector of the fenestrated tracheostomy tube (e.g.
TRACOE twist, TRACOE twist plus or TRACOE comfort tube).
The valve can be removed without difficulty from the button located
in the tracheostoma or from the TRACOE twist cannula while still
in place. The valve is removed either when the patient no longer
wishes to speak or when secretions have collected in the valve and
this needs to be cleaned.
4.2
General Requirements for Use
of the Valves are as follows:
adequate spontaneous breathing in the conscious patient
stable cardiovascular conditions
ability to swallow (at least partially present)
no excessive bronchial secretion
blood gas values in the normal range,
with oxygen supply, if necessary
These requirements apply both when the valves are being used for
speaking and when weaning the patient from a tracheostomy tube.
CAUTION: if, when speaking or during weaning, the patient
becomes breathless or air becomes trapped, the lateral apertures
are opened by turning the valve head anticlockwise as required. This
disables the one-way valve function and the patient can breathe in
and out without difficulty, without having to remove the valve.
4.3
Speaking with the Valve
For speaking, the lateral apertures of the valve are closed by rotating
the valve head fully clockwise. This allows air to enter only during
inspiration. During expiration, the valve closes and the air flows out
through the larynx, throat and mouth, allowing voice production.
