Implant the Cylinders
Use the Furlow Insertion Tool to introduce the cylinders into the corpora cavernosa,
as follows:
1. When inserting the cylinder distally, use the Furlow Insertion Tool to put the penis
on a mild stretch. The tool should be palpable in the glans.
2. To help prevent cross over through the intercarvernous septum when placing the
first cylinder, a dilator or other surgical tool may be placed in the contralateral
corpus cavernosum.
3. Withdraw the obturator of the Furlow Insertion Tool into the retracted, or
locked position.
4. Thread both ends of the traction suture (3-0 braided, nonabsorbable, PTFE coated
polyester) at the distal tip of the cylinder through a Keith Needle (Figure 9), and
place the blunt end of the needle in the insertion tool (Figure 10). Place the suture
in the tool slot, completely retract the suture into the slot and fully draw the needle
into the barrel of the tool.

5. Hold the four strands of suture against the tool and insert the tool into the distal
portion of the corporal body until the front tip is at mid glans.
a. During insertion, be sure that the Furlow Insertion Tool is in the ipsilateral corpora
at the distal penis. Because the intercavernous septum may be inconsistent distally,
it is easy to cross over to the contralateral side.
b. If you think you may have crossed over, remove the Furlow Insertion Tool, place
a dilator into the contralateral side and reposition the Furlow Insertion Tool on the
ipsilateral side. No repair is necessary.

6. Pass the needle through the glans by fully inserting the obturator into the barrel
(Figure 11). Grasp the needle with a needle holder and pull it completely through
the glans.

7. Detach the needle from the suture, and remove the needle from the area to prevent
any accidental cylinder punctures. Attach a hemostat to the remaining suture to
prevent retraction through the glans.
8. Repeat the procedure using the remaining cylinder.
9. Inflate the cylinders fully, then insert the proximal end of the first cylinder into the
crus (Figure 12). The Proximal Tool may be used to place the proximal cylinder end
into the corpora.

10. Deflate the cylinder by bending it to approximately 65 degrees and holding it for
10-12 seconds (Figure 13).
