Connection to HF unit in monopolar mode
First fix the neutral electrode ("return plate") to the patient,
preferably on the upper arm or on the thigh. Ensure that the
skin is free from hair and grease. If a reusable neutral
electrode (return plate) is used, do not apply conductive
gel. The current paths inside the patient's body should be
short and must run diagonally. Current paths must never
run in a transverse direction through the body or across the
thorax.
The patient must be insulated against all electrically
conductive parts. Ground the operating table. Place the
patient on a dry electrically insulating layer. Prevent skin
areas touching each other. Place dry gauze in between.
Then connect the neutral electrode, the foot switch and the
"active" HF cable to the HF unit.
In order to avoid interference with camera systems, do not
run HF cables directly in line with camera cables.
Never place HF cables directly on the patient's skin. Never
lay HF cables in loops. Next, connect the instrument or
handle with coagulation electrode, and switch on the HF
unit. The electrode with the proper shaft diameter (4 mm or
2.4 mm) must be firmly attached to the handle tip.
For handles with finger switches:
Yellow switch = actuation of cutting current.
Blue switch = actuation of tissue coagulation.
Always refer to the instruction manual of the HF unit and to
the general information it provides relating to
electrosurgical procedures.
Connection to HF unit in bipolar mode
In bipolar HF surgery the coagulation is restricted to the
area between the two electrodes of the bipolar instrument.
Consequently, a neutral electrode is not necessary.
