Transbuccal insertion of the first pair of pins
To facilitate transbuccal pin insertion, use the pin guide (4) and the twin trocar (3, see
section 10.3 "Instruments", page 40).
In a first step, insert the self-tapping pin (1) located closest to the planned osteotomy line.
Use the pin screwdriver (item no. 51-600-85, see section 10.3 "Instruments", page 40) to
fix the pin in the bone.
Now the pin (2) farther away from the osteotomy line is inserted and fixed in the bone,
making sure that the pins are fully parallel. In the case of very dense bone, we recommend
drilling a pilot hole using a 1.5-mm drill bit.
Attaching the distractor and inserting the second pair of pins
Preliminary fixation of the distractor allows you to adjust the distraction vector on the
distractor body to the anatomical conditions of the patient. Vector adjustment is possible
across a range of ±20° in transversal direction and -6° to +98° in vertical direction.
Adjustments are made with the vector adjustment screwdriver (item no. 51-600-80-07,
section 10.3 "Instruments", page 40).

3DX External Distraction System
It is advisable to lift the skin or push it a
little upwards prior to making the incision,
as this allows you to insert the pins in the
skin region that usually comes to rest in the
submandibular fold. In this way, potential
scars remaining after distraction will be
limited to an inconspicuous area.
Make two small transbuccal stab incisions
with a distance of one centimeter between
them.
The distractor is now connected to the
previously inserted pin pair and temporarily
fixed in order to determine the exact
position of the second pair of pins. To do
this, you first need to loosen the pin fixation
screw on the pin holding clamp using the
vector adjustment screwdriver (item no. 51-
600-80-07). This exposes the cutouts (7)
that function as perfect seats for the two
pins inserted parallel. These cutouts are
used to connect the distractor to the pins.
To fix the pins in place, retighten the pin
fixation screw (6) slightly with the vector
adjustment screwdriver. See also
section 10.3 "Instruments", page 40).
