Meals – The patient can record meals, meal size (small, medium, or large), and
how many carbs were eaten. The patient can take pictures of the meals eaten to
review and discuss with the HCP. The pictures will not be uploaded to the final report,
but the patient can review them with the HCP on the patient's mobile device. When
the app is deleted, meal pictures will be deleted.
Insulin – The patient can enter insulin type and the units delivered.
Exercise – The patient can enter any exercise and indicate whether the exercise
was light, moderate, or intense, and for how long you exercised.
Medication – The patient can enter any non-insulin medications, pills, or
injectables taken during the evaluation.
Sleep – The patient can enter when they went to sleep and when they woke up.
The patient can add notes about their quality of sleep.
Notes – The patient can enter any other information, such as blood glucose
readings or any hypoglycemic or hyperglycemic episodes.
Navigating through My Journal
The following are steps the patient can take for adding, reviewing, editing, and
deleting multiple events in My Journal.
• To add meals, insulin, exercise, medication, sleep, or notes, the patient can tap
the button at the bottom of the screen and choose the event they would like
to add.
• To review events, the patient will select the event they would like to review.
• To edit events, the patient will select the event to edit, tap Edit, make changes,
and tap Save.
• To edit events on a previous day, swipe on the My Journal screen.
• To delete events, the patient will select the event to delete, tap Edit, and scroll
down and tap Delete Event.
• For additional information, select the Menu icon located at the top left corner of
the screen.
End evaluation and send data
At the end of the seven-day evaluation, the patient will receive the notification, "Your
Evaluation Ended!" The patient can end the evaluation and send the data using the
app by following the instructions on the screen, or they can bring their mobile device
and recorder to the HCP to end the evaluation. Make sure the patient uses the app to
