The HCP will tap Clinic ID and manually enter their Clinic ID number and tap
Find Clinic.
OR
The HCP will tap Scan QR Code to use the mobile device camera. The HCP
will hold the phone over the previously printed QR code. The app uses the
mobile device camera to scan the QR code.

The HCP will verify their clinic name and address, and tap Confirm.

Review the End User License Agreement with the patient. If the patient consents,
they will tap I Agree.

Review the Patient Consent with the patient. If the patient consents, they will tap I
Agree.

The patient and HCP will review voluntary consent, if applicable for the patient's
region. After reviewing each voluntary consent, the patient will select Yes or No
and select Next to continue.

The patient will now create a patient account. They will enter their e-mail address.
They will confirm their e-mail address.

The patient will create a password following the rules indicated on the screen.
They will confirm the password.

The patient or HCP will enter the First Name, Last Name, and Date of Birth.

The patient or HCP will select their Diabetes Type, for example Type 2. This
step is optional.

The patient or HCP will select their Therapy Type, for example Oral
Medication(s). They will enter all that apply. This step is optional.

Enter the patient ID and physician name. This step is optional.

Tap Save. Review the information and tap Confirm if accurate.

The patient will enter the email address and password they used to create their
account. Tap Login.

The patient and the HCP will perform the following steps to link the app and pair the
