Introduction
Positive Expiratory Pressure (PEP) therapy has been shown to be beneficial for opening of
airways in the lungs, and can assist with secretion clearance. It may also be used as a
substitute for pursed-lip-breathing in order to promote muscle strengthening and endurance.
Such therapy requires a controlled breathing technique that results in an increase of air
pressure in the lungs when exhaling. One effect of such an increased pressure is that air and/
or medicated aerosol can be pushed slightly deeper into the lungs, where it may help to loosen
secretions, which can then be coughed up. The recommended difference in pressure when
inhaling and exhaling with the PARI PEP™ S System is in the range between 10 and 20 cm of
water. The proper resistance setting on the device and the patient's breathing technique should
be determined and monitored by a physician or therapist with the use of a manometer. Selection
of the proper resistor setting produces the desired inspiratory-to-expiratory ratio of 1:3 to 1:4.

Indications for Use

The PARI PEP™ S is intended for use as a Positive Expiratory Pressure Device, and is designed to
help patients exercise their lungs properly and improve secretion clearance. The PARI PEP™ S
may be used by itself or in conjunction with aerosol drug therapy. The PARI PEP™ S is intended
for adult and pediatric patients, for use in home, hospital, and sub-acute institutions.

Contraindications

Although no absolute contraindications to the use of Positive Expiratory Pressure (PEP) Therapy
have been reported, the following should be carefully evaluated before a decision is made to
initiate therapy. Inability to tolerate increased work of breathing; intracranial pressure (ICP)
> 20MM Hg; hemodynamic instability; recent facial, oral, or skull surgery; acute sinusitis;
epistaxis; esophageal surgery; active hemoptysis; nausea; known or suspected tympanic
membrane rupture or other middle ear pathology; and untreated pneumothorax.
