4. THE OPERATION

Positioning of the trephine:
1. Check that the suction cone is firmly screwed onto the
main body of the trephine.
2. Mark the center of the trephination either with an
indentation or with a methylene blue surgical pen. Dry
the cornea and the conjunctival cul-de-sac. The trephine
is held in one hand while the other grasps the external
rectus with tweezers to center the trephine guide on the
corneal mark.
3. Press the trephine on the eye while the assistant
applies suction with the syringe. Verify the vacuum of
the trephine on the eye by pulling gently upwards.
Trephination:
1. Turn the adjustment knob clockwise.
2. When the Descemet's membrane is cut, a small
discharge of aqueous humor appears (incomplete
penetrations are possible by setting lower incision
depths). Stop turning the adjustment knob and have the
assistant release the suction to remove the trephine.
End of the operation:
1. After trephination is complete, withdraw the blade, turn
the knob counter-clockwise, and lock it.
2. Unscrew the cone. Unlock the locking collar and remove
the blade. Dispose of the blade in an appropriate
container.
3. With the key (B), turn the ratchet collar (J) counter-
clockwise in order to return to «0».

5. TROUBLESHOOTING

If the device does not work properly, make sure there is no visible defect.
o
In the event of suction problems, check the integrity of the tubing and change it if necessary. Also
check that the holes in the suction cone are not obstructed.
o
If the blade cannot be inserted properly, make sure that the guide has been removed (see
precautions for inserting the blade page 3). If the blade depth cannot be set check for the presence
of the guide.
o
If the device is locked, make sure that it has been returned to «0» using the ratchet collar (see.
«end of the operation - precautions» in the table above).
o
In the case of cutting problems, check that the suction cone is securely screwed onto the main
body of the trephine.
o
If the trephine is blocked, remove the blade completely from the trephine and lock the adjustment
knob (turn the adjustment knob counter-clockwise until it stops, then turn it 90° clockwise and lock
it). This process should be systematically carried out with the ratchet collar before each change in
position.

PRECAUTIONS
Other than the weight of the trephine itself, the surgeon
must not exert any pressure on the eye during the operation.
Warning! Failure to withdraw the blade will seriously
damage the trephine and its blade depth regulation
mechanism.
Returning to the «0» position is compulsory (V).
If not carried out, there is a high risk of jamming the
system.
