Service and Claims:
If service is needed either during or after the warranty period:
• In the United States, contact SurgiQuest, Inc.
at +1-414-434-6634 or call your local SurgiQuest, Inc. sales representative.
• Package all the components carefully in the original shipping container. If the
original shipping container is not available, an equivalent shipping container
must be obtained from SurgiQuest.
• Print the Return Authorization supplied by SurgiQuest, Inc. and prepare for
shipment pickup at scheduled time and location.
