Limited Warranty Coverage
The information contained in this section (such as separation
distances) is, in general, specifically written with regard to the
OmniPod System. The numbers provided will not guarantee
faultless operation but should provide reasonable assurance of
such. This information may not be applicable to other medical
electrical equipment; older equipment may be particularly
susceptible to interference.
Insulet Corporation (the manufacturer of the OmniPod® system)
warrants to the original end-user ("you") that the Personal
Diabetes Manager (the "PDM") will be free from defects in
materials and workmanship, under normal use and conditions,
for a period of five (5) years from the date of delivery. If Insulet
Corporation determines that the PDM is defective during the
warranty period, Insulet Corporation will either repair or replace,
at its option, the PDM at no charge to you, subject to the
conditions and exclusions stated herein. This Warranty applies
only to new devices and, in the event the PDM is repaired or
replaced, the warranty period shall not be extended.
Warranty Claim Procedure
You must notify Insulet Corporation of the claimed defect within
the warranty period by calling the Canadian customer care line
at: 1-855-PODINFO (763-4636) maintained by the Canadian
distributor of OmniPod® in Canada, Insulet Canada Corporation.
The claim must include the date of purchase, model number,
serial number, and a description of the claimed defect. Insulet
Corporation's authorization must be obtained prior to returning
the PDM. If authorized, the PDM must be properly packaged and
returned to Insulet Corporation. Insulet Corporation will pay all
freight and transportation charges, where applicable, incurred in
shipping a PDM to be repaired or replaced under this Warranty.

Conditions
Proof of Purchase in the form of a bill of sale or receipted invoice
showing that the PDM is within the warranty period must be
presented to obtain warranty service.
Exclusions
This Warranty covers only the original end-user and cannot be
transferred with sale, rental or other transfer of the PDM to any
other person or entity.
