Milk not fresh.
Milk not suitable.
Milk was too warm
from the start.
Not enough milk to
froth.

Remedy
Use fresh milk.
Use lower fat content
dairy milk. Non-dairy milk
won't froth as
well.
Use milk straight from
the refrigerator (34°F).
Use more cold milk.
