At the end of installation and testing it is declared:
∗ Installation has been properly carried out;
∗ The machine is fully functioning;
∗ All the guard devices are present and fully operational;
∗ The Installer has provided all the information required for proper running, use and
maintenance of the machine;
∗ The Installer has provided all the information required for proper accident
prevention;
∗ The user manual is present.
For the Purchaser
This declaration is invalid if it is not properly filled in and signed. Copy for the
Installer to be sent to the Manufacturer.
The Purchaser and the Installer who fill in this certificate, hereby authorise the manufacturer of the machine to process the information provided herein, as required to
manage the guarantee and ensure traceability of the product, under Legislative Decree 196/2003 "Safety of personal information" (Privacy).
