8. Auscultatory blood pressure
measuring
8.1. Basic rules for the patient support
The patient should:
• rest for 5 minutes before the first measurement
• sit comfortably
• do not cross their legs
• lean or otherwise support the back and arms
• be as relaxed as possible during the blood pressure
measurement and should not talk
8.2. Preparation
1) It is recommended to use the following phases of Korotkoff
sounds for auscultatory measurement:
• in adults, phase V (K5)
• in children aged 3 to 12 years, phase IV (K4)
• in pregnant women, phase V (K5), unless noises are heard
with the cuff deflated, in such cases phase IV
(K4) is used
K5 is the point at which the sounds heard with a stethoscope
are no longer audible
K4 is the point at which the sounds heard with a stethoscope
vary from clear tapping to muffled.
2) Before taking a measurement, insure that the blood
pressure measuring device and the cuff are in perfect
working order.
3) The cuff must be chosen to fit the arm circumference:
Arm cuff
Infants
Children
Adults
Large adults
Arm circumference
13.8 - 21.5 cm
20.5 - 28.5 cm
27.5 - 36.5 cm
35.5 - 46.0 cm
4) A stethoscope is required separately for auscultatory
measurement.
5) Help the patient to relax and remind them not to speak or
move during the measurement.
8.3. Attaching the arm cuff
1) The cuff must not be applied over wounds as this may cause
further injury.
2) Blood pressure must always be measured on the arm with
the higher pressure values. For this purpose, measure the
blood pressure on both arms first and then always on the
arm whose blood pressure value is higher.
3) Pull the cuff up over the upper arm. Thereby the tube must
be pointing downwards. Make sure that the center of the
cuff on the upper arm is at the same level as the right
atrium of the heart.
4) The artery sign ɸ on the cuff (KaWe logo visible from the
outside) must be on the inside of the upper arm in the area
of the artery.
5) Tighten the cuff firmly but not too tightly (leave 1-2 cm
free) and lock it with the Velcro fastener.
8.4. Performing the measuring
1) Close the release valve on the pressure ball by turning the
screw to the right.
But do not overtighten the screw.
2) Pump up the cuff with the pressure ball quickly and
rhythmically until it exceeds the systolic pressure.
But never inflate to more than 300 mmHg!
3) Open the release valve slowly so that the pressure drop at
the pressure gauge is approx. 2-3 mmHg/s.
Read the systolic and diastolic pressure perpendicular to
the scale to avoid reading error.
4) After the measurement is finished, open the release valve
fully to let the air escape completely from the cuff.
5) Remove the cuff from the arm.
6) Stow the gauge and/or the cuff:
• MASTERMED A series: in the bag supplied
• MASTERMED C series: in the holder
